The 4-in-1 program ensures culturally acceptable personal and public health services are available and accessible to Urban Indians. This program focuses on 1. diabetes, 2. immunizations, 3. alcohol- and substance use-related, and 4. mental health services. Funded by the Indian Health Service.

Case management – Staff provides case management services to discuss High Blood Pressure and Disease Preventions services such as diabetes, home visits and immunizations as needed.
