Achieving AAAHC Accreditation reflects that SDAIHC has met the rigorous standards of a nationally recognized third party. Our organization embraces the “1095 Strong” philosophy, ensuring ongoing improvement and quality care throughout our accreditation term.

SDAIHC is accredited by the National Committee for Quality Assurance as a Patient-Centered Medical Home (PCMH), reflecting our commitment to providing evidence-based whole-person care.

The PCMH model ensures that each patient receives the best possible care when they need it, in a manner they can understand. Our team of providers coordinates care across multiple settings, paying special attention to each individual’s culture, values, and preferences. We work in partnership with patients and their families, empowering them to make healthy lifestyle choices.

As a PCMH, we continuously evaluate our services and health outcomes to guide quality improvement. We encourage patients to understand their role in a PCMH by reviewing the Patient Rights and Responsibilities form and the Notice of Privacy handout, both available on our website and in our health center.
